Membership Applications and Renewals

Step 1:  Download form and fill it in.

Step 2:  Choose payment option.

Step 3:  Send form to Membership Secretary.

1. Download form

Click on the link above.  If you wish to send the form by email, fill it in and save it.  If you wish to post the form or hand it to the Membership Secretary, you may print the saved file or print the blank form and fill it in by hand.  A form filled-in by hand may also be scanned or photographed, converted to PDF then emailed.

If you are renewing your membership and paying by bank transfer or deposit, you do not need to fill-in a Renewal form unless some of your details have changed.

(You may need ‘Adobe Reader’ or ‘Adobe Fill & Sign’ apps installed to download and fill-in the form.)

 

 2. Payment options

Bank transfer or deposit –  this is our preferred method of payment
Use online banking or visit any bank branch and transfer or deposit the Membership Fee into our bank account:

Account Name = Life Activities Club Knox Inc.
BSB = 033372
Account No. = 106667

If depositing at a bank branch, please ask the teller to include your membership number in the Description field.  You may use the Bank Payment form to make it easier.
If transferring online, please put your last name and membership number in the Description field.

All payments must be made individually.  However you may pay for a spouse or partner as well in one transaction by adding 999 at the end of your membership number.

Important:  Once you have transferred or deposited the funds, please write the Date Paid in the appropriate box on the Membership Application or Renewal form.

Cash –
Cash notes are acceptable if you wish to hand the form to the Membership Secretary.

Personal Cheque, Bank Cheque or Money Order –
Please make these out to Life Activities Club Knox Inc.

 

3. Send form

If you are renewing your membership and paying by bank transfer or deposit, you do not need to fill-in a Renewal form unless some of your details have changed.

Email –
Attach the completed form to an email.  Please add a Subject beginning with your name, membership number(s) and text “Membership Application” or “Membership Renewal”.  Send the email to the Membership Secretary at:  knox.membership@life.org.au

Post –
Enclose the form in an envelope and post it to:
Membership Secretary, Life Activities Club Knox Inc., PO Box 19, BORONIA VIC 3155

Important:  Do not send cash by mail.

Hand –
Enclose the form in an envelope and write your name and membership number on the envelope followed by text “Membership Application” or “Membership Renewal”.  Enclose the Membership Fee if you have not transferred it via the bank.  Hand it to the Membership Secretary.  The envelope may also be handed to any Committee Member or Activity Group Convenor.
